This position will be one day/week (Mondays) at Rush Chicago.

The Home Care Liaison coordinates care for patients requiring health care in the home. Serves as a resource for the hospital staff and patients concerning services available, reimbursement requirements and related areas in discharge planning. Responsible for utilization review activities taking the appropriate action as needed. Also responsible for evaluating intravenous needs and appropriate venue post hospital discharge and coordinating those services with contracted vendors.

Specific Responsibilities:

Acts as a resource to the hospital staff concerning services available, reimbursement requirements and planning which needs to be completed prior to discharge. Attends and participates in hospital in-service programs and patient conferences related to patient care and discharge planning. Supplements teaching to patient and caregivers as needed to assist with discharge planning.

Serves as the home care representative and presents services to the patients as per physician orders. Explains home care services and informs the patients as to what they can expect. Confirms the patient demographics as well as emergency contacts so care can be coordinated.

Collaborates with physicians and the health care team to identify patient’s needs for home care services, community services and medical equipment. Makes a recommendation to physicians for discharge planning based on the assessment and coordinates and/or arranges for services and/or equipment needs for the final discharge location as necessary.

Serves as the contact point for the Home Care Department personnel for current home care patients brought into the Emergency Department and/or admitted into the hospital.

Coordinates and arranges for durable medical equipment as well as the pharmacy for infusion therapy supplies and medications in a timely manner as needed.

Maintains contacts in hospitals and doctor’s offices for obtaining referrals. Establishes and develops relationships within the community. Identifies patients that are appropriate for home health services and makes recommendations regarding same patients.

Assists with the orientation and training of new personnel as required


Current licensure as a Registered Nurse in the state of Illinois
Completion of an Associate’s degree from an accredited school of Nursing. Bachelor’s Degree highly desirable.
Minimum of two (2) years of previous nursing experience preferably in home health or community health setting.
Current CPR certification.
Current Illinois driver license and auto insurance liability.
The communication and interpersonal skills necessary to regularly interact with all levels of hospital personnel, physicians, patients and family members in a professional manner.
Extensive knowledge of insurance plans, the criteria for home health and of infusion therapy.
The analytical ability to identify potential referrals. Coordinates the assignment of agency personnel and to exercise clinical and professional judgment in providing comprehensive care to patients.
Ability to work weekend rotations and travel to various locations within the Vanguard Chicago Market.
Working knowledge of Microsoft Office applications and office equipment.
