The Nurse Practitioner assumes total nursing care responsibility of specific patients under the direction of a physician. Follows established nursing standards, procedures, and practices and gives specific patient care directions to nursing and other staff.


Consults with the House Call Physician(s) concerning the patient’s evaluation. Receives assigned patients and orients them to house call program. Instructs patients and families and answers questions.

Provides diagnostic and assessment services and secures a health history from the patient and/or the family, records findings and critically evaluates them. Performs a complete in-depth physical examination. Uses assessment techniques including observation, inspection, auscultation, percussion, palpation and oto-ophthalmic examinations. Requisitions laboratory tests and X-ray examinations when appropriate.

Provides therapeutic services and confers with other health care providers to prepare a comprehensive patient care plan. Identifies and manages specific minor illnesses.

Participates in the planning and evaluation of nursing and medical care policies and procedures of the clinic

Provides preventive health care services to patients and guides patients in nutrition, common and non routine illnesses and accidents. Assists in routine immunizations, vaccinations of patients against influenza and other epidemics where indicated. Teaches patients and their family members ways to improve or maintain their health status. Participates in formal and informal patient-teaching activities aimed at promoting positive health practices and an understanding of the individual’s medical problems

Arranges consultations and referrals and initiates requests for consultations or referrals. Responds to requests from the patient, physician or nursing staff. Examines the patient and record, and discriminates between normal and abnormal findings to recognize early stages of serious physical, emotional or mental problems and determine when to refer the patient to a physician for evaluation, supervision or directions.

Maintains required records, reports and statistics. Maintains accurate patient records, charts and documentation for administrative purposes and reimbursement for services.

Triages telephone requests for emergency appointments to control utilization and appropriately schedule patients.

Confers with physician(s) concerning assigned patients and keeps physician(s) informed as appropriate of the needs and problems related to the patient’s care.

Complies with the established nursing standards. Implements and maintains established department policies, procedures, objectives, quality assurance, safety, environmental and infection control. Interprets these to physicians, patients and the public as is appropriate. Utilizes supplies and equipment in a cost-effective manner. Maintains a clean, orderly and safe environment for patients and other personnel.

Assists with the care and maintenance of equipment, supplies and work areas. Requisitions supplies and equipment as needed for patient care. Requests maintenance and repair of malfunctioning equipment. Disposes of defective supplies.

Maintains clinical bag carrying necessary equipment. Utilizes proper infection control techniques.

Cooperates with other personnel to achieve department objectives and maintains good employee relations, interdepartmental relations and public relations. Maintains patient rights and confidentiality of patient information and abides by the established communications codes

Attends required meetings, in-services and educational programs.

Maintains their professional growth and development through attendance at seminars, workshops and professional affiliations.


1. Current licensure as a Registered Nurse in the state of Illinois.

2. Completion of Masters Program from an accredited school of Nursing.

3. Completion of an accredited program for nurse practitioners including preceptorship.

4. Current APN certification.

5. Current CPR certification.

6. Professional competence documented by references based on nursing experience, education or both.

7. The analytical ability necessary to assess a patient’s condition and utilize the nursing process and professional judgment in providing comprehensive care to patients.

8. The interpersonal skills necessary to constantly interact with patients, families, medical personnel and other members of the health care team.

9. The ability to perform effectively and efficiently under the pressure of stressful or emergency situations.

10. Ability to drive and visit patients at home.
