The Social Worker objectively and systematically assesses, monitors, evaluates and improves the processes that affect the quality of patient care, ensuring compliance with: MacNeal Health Network mission and values, The Care Management Program, rules and regulations, MacNeal policies and regulatory standards.

Utilizing established practice and professional standards, this position assesses and provides intervention concerning disability resulting from emotional, social and economic stresses of illness; identifies and provides interventions to meet the psychosocial needs of patients and their families; resource for complex discharge planning needs.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

1. Assessment/Intervention
a. Demonstrates an advance level of analytical and clinical judgment in assessing
patient/family emotional, social and economic stresses of illness; and in
patient/family coping abilities and capacity for change.
b. Establishes priorities for intervention; develops plans for intervention.
c. Provides crisis intervention to assist patient/families.
d. Facilitates patient/family participation in self care based on age and developmental status for both pediatric and adult populations.
e. Demonstrates appropriate psychosocial interventions based on age and developmental status for both pediatric and adult populations.

f. Provides patient/family education; assists patient/family in post-hospitalization care
planning; management of financial issues while facilitating discharge planning; makes
referrals to community agencies and/or residential facilities as necessary.

2. Discharge Planning
a. Determine availability of needed resources against benefit options and patient preferences
b. Communicate multiple options to patient/family and physician to prevent discharge delays
c. Demonstrates expertise in all complex discharging issues; handles all cases related to guardianship, abuse/neglect, domestic violence, adoptions, psychiatric transfers, addiction referrals and financial issues.
d. Manages other discharge needs in conjunction with case managers.
e. Implement plan and evaluate effectiveness
e. Facilitates transitioning patient through continuum of care

3. Performance Improvement
a. Assists in the development and implementation of action plans to improve quality and reduce costs.
b. Working knowledge of performance improvement methodology and applies the principles to improvement projects
c. Understands the fundamentals of root cause analysis and participates in identification of system issues
d. Tracks and trends data over time for informed decision making
e. Assists in the development and analysis of critical pathways
f. Investigates and communicates identified potential quality or risk issues
g. Participates in data collection for defined organization or physician specific indicators

4. Care Coordination
a. Identifies highest risk patients that would benefit from focused case management.
b. Identifies delays in service.
c. Participates/conducts patient/family conferences
d. Provides consultations regarding advance directives; assists in completion when needed.
e. Demonstrates an understanding of ethical issues; facilitates ethics consultations as indicated.
f. Evaluates environmental safety and provides an environment that is safe based on age and developmental status for both pediatric and adult populations served to maintain compliance with National Patient Safety Goals.
g. Resolves problems and difficulties encountered in patient care and planning, along with other members of the multidisciplinary health care team.
h. Provides social work intervention in the Emergency Department on an on-call basis as assigned.

Qualifications:

1. A Master’s Degree in Social Work.

2. Minimum of two (2) years of social work experience in a hospital setting.
3. Current certification by the State of Illinois as a Licensed Social Worker. (L.S.W.) or LCSW.
4. Familiar with Joint Commission standards.
